I will check my foster dog regularly for any signs of illness. I will do frequent flea and tick checks on my foster dog.

I agree to care for the dog in a humane manner. This includes adequate food, water, shelter, attention, and safe environment and advising Saved By Grace Dog Rescue, Inc. representatives if pet needs medical care.

I understand that I will not incur any expenses related to veterinary care. If I incur expenses related to the dog without prior consent from Saved By Grace Dog Rescue, Inc. I understand that I am personally responsible for those expenses.

I will never leave my foster dog unattended in a vehicle when it is 75 degrees or hotter, even with windows Rolled down. I understand this can cause a heat stroke.

I will closely supervise this foster dog around children and adults (especially ones the dog is not familiar with). I will closely monitor when the foster dog is around roadways or water, riding in a vehicle, and any other potential dangerous situations. I will make sure the dog will wear identification tags at all times. If I do not have identification tags I will ask how to obtain such. If my foster dog escapes I will agree to inform Saved by Grace immediately and try to locate the foster dog.

I agree that if at any point I decide I can not keep the foster dog for any reason, I will return him/her to Saved By Grace Dog Rescue, Inc. and allow 24 hours for pick up or return myself to the agency.

I understand Saved By Grace Dog Rescue, Inc. has little to no knowledge of the dog including medical, background and disposition. Saved By Grace Dog Rescue, Inc. makes no warranties with regard to health or temperament and disclaims any liability for such.

I release and forever discharge Saved By Grace Dog Rescue, Inc. , its board, volunteers, foster homes, and veterinarians treating the dog at the request of Saved by Grace from any and all claims and actions including but not limited to property damage caused by dog, personal injury to me, family members, or third parties or other animals from biting, scratching, disease transmission and/or any other matter attributed to said dog.

I acknowledge that Saved By Grace Dog Rescue, Inc. is an Oklahoma non-profit organization and any disputes or disagreements arising from my dealings with Saved By Grace Dog Rescue, Inc. are subjected to Oklahoma law and I consent to the exclusive jurisdiction of Oklahoma law. If Saved by Grace is forced to retain an attorney in enforcing this contract recipient agrees to pay Saved By Grace Dog Rescue, Inc. reasonable attorney's fees and costs incurred in same.

I understand and agree to the above terms. If, at any time I am unable to care for the foster dog according to the conditions stated above I will allow Saved By Grace Dog Rescue, Inc. to resume control of the animal or an alternative solution. I understand that if I knowingly fail to comply with the above terms, or put the foster dog in a situation that endangers its health, Saved By Grace Dog Rescue, Inc. may take appropriate action, including legal action, to take the pet from my possession.
